Note 1: These are the same file formats submitted to the FLDOE for FTE.
No data elements need to be removed, but the files must contain the listed fields.
Note 2: The Local Educational Agency (LEA) is responsible for submitting data for participating charter and non-public schools.
Note 3: The Responsible Instructor: Reading file is a modified version of the Responsible Instructor: Math, Reading, and Writing format used for grades 3-12.

Note 4: The responsible reading instructor is NOT always the homeroom teacher.

- at secondary schools, for students who scored a Level 1 or Level 2 on the reading portion of the previous year's FCAT, the responsible instructor of reading is the teacher of the intensive/supplemental reading class. This might be a content area course such as social studies or science. For FCAT L1 and 2 students, it is not the language arts teacher.
- for FCAT Level 3, 4, or 5 students, at secondary schools, the language arts teacher is the responsible instructor of reading.
but the elementary teacher who provides the initial 90-minutes of uninterrupted reading instruction or the secondary school teacher who provides the intensive reading instruction.
